Required Athlete Paperwork
Interested in playing a sport at YMS?
We have chosen to use a web-based program for participation in sports here at Yearling Middle School. Below or on our Yearling Middle School website https://yms.okee.k12.fl.us/ are what you need to get started.
First, go to the website: www.nfhslearn.com (Click this link for a quick instructional video).
Create an account. (you will need to have an active non-school email account).
Schedule and have doctor complete FHSAA Physical (EL2). Keep the original copy for your records. Scan copy as a PDF for Google Docs.
Complete the nfhs.learn courses below and print/save digital certificates. Keep the original copy for your records. Scan copy (if needed) as a PDF for Google Docs.
Complete (Sign and Date) Consent and Release from Liability Certificate (EL3) Keep the original copy for your records. Scan copy as a PDF for Google Docs.
Parents/guardians and athlete upload completed PDF documents into Google Forms. Don't forget to keep the original copies for your records.
If there are problems completing any part of the sport sign-up, please contact the coach first. Oftentimes, office personnel do not have the answers.
*Pediatricians can take weeks to get appointments scheduled but are usually free. Chiropractor/Urgent Care appointments can sometimes be scheduled and completed same day but cost ranges from $25-$100.
**Parents/athletes may find it easier to use their phone to scan documents. However, athletes can come to the Main Office to scan documents if needed.
